Confidentiality / Release of Information

All medical records and clinical consultations are strictly confidential. Records cannot be released without the written consent of the patient except in the event of a life-threatening medical emergency, or in response to a subpoena, or for reporting purposes as required by law.

Authorization for Release of Medical Information forms can be obtained and completed at The Infirmary.
